eA statement from you that you have a good faith belief that use of the copyrighted work is not authorized by the copyright owner, its representative, or the law.
fA statement from you that this information is accurate, and under penalty of perjury, that you are authorized to act on behalf of the copyright owner.

Velvet Klub works in conjunction with copyright holders to prevent unauthorized use of their creations. Velvet Klub specifically reserves the right to seek compensation for any damages sustained as a result of any fraudulent activity.
